Patient Forms
If you are a new patient to our office, please visit our PATIENT FORMS page to download our new patient bundle forms. You will need to arrive 20 minutes early to fill the forms out in our office. Thank you and please contact our office if you have any questions at all.
Payment Options
For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed.
Dental Insurance
Dr. Sharon is a provider for Delta Dental. We welcome all PPO insurance plans. Please contact our office for insurance details. We look forward to hearing from you.
Private Care Dental Savings Plan
We also offer a Private Care Savings Plan. This alternative to dental insurance provides 2 dental cleanings and 2 dental exams per year, as well as radiographs, oral cancer screening and flouride treatment. It also includes a 10% courtesy on dental procedures.
